ACO REACH: Advancing Equity and Optimizing Performance

CMS is replacing the Global and Professional Direct Contracting (GPDC) Model with the redesigned ACO Realizing Equity, Access, and Community Health (REACH) Model in 2023. ACO REACH is the first accountable care model to directly address health equity and access to care, with a specific directive to meet the needs of patients from marginalized and underserved communities. Whether you are participating in ACO REACH, the model provides vital insights into the future of value-based care and care collaboration.

This panel will discuss the incentives and requirements laid out by the new ACO REACH model and how organizations can develop action plans to identify differences or disparities in their members’ health status.

   

Gary Jacobs

Gary is a seasoned healthcare executive with a wide breadth of experience in the government programs’ market and a concentration on Medicare Advantage, Medicaid, Medicare Supplement, long-term care, public and private exchanges, individual products and payer/provider collaborations. He has a successful history of developing, selling, and acquiring healthcare companies. Recognized for quickly assessing the big picture and implementing workable plans to increase revenue and profitability targets, Gary keenly understands public policy and its role in influencing a program’s profitability and ultimate success.

Ashley Perry, MPH

Chief Solutions Officer, Socially Determined

Ashley manages a multidisciplinary team that translates Social Risk Intelligence™ into quantifiable business results and community impact. As a public health practitioner, researcher, and management consultant, she has bridged healthcare and public health. Prior to Socially Determined, she built and scaled successful consulting practices for the Premier Healthcare Alliance and Kaufman Hall. Ashley also previously led research and programmatic initiatives for organizations ranging from an academic research center to a network of federally qualified health centers.

 

She holds a Bachelor’s degree in Economics from Duke University, a Master’s degree in Public Health from Columbia University and is a certified Public Health professional. In 2020, Ashley was recognized by Modern Healthcare as a Top 25 Emerging Leader and “one of the most impressive leaders driving change across the industry.”

Kate Casaday

Director of Market Operations, CareMount

Kate Casaday began her career working on hospital staffing models at The University of Chicago where she studied Cancer Biology.  Other experience at that time includes work as an ocean rescue diver in New Jersey and first responder.  Kate completed graduate studies in Public Health at Columbia with a concentration in sociology, and after a stint working fine-dining in Manhattan started work on healthcare payment models with the insurer, Emblem Health.  Kate’s expertise developed using game theoretical concepts to understand market dynamics and prevailing stakeholder incentives. She transitioned from insurer to the provider side in 2018 and began work on the Next Generation ACO Model with Care Mount Health Solutions.  At CareMount Kate focused on risk adjustment processes for traditional Medicare, value-based care change management, and provider incentive design.  After acquisition in 2020, Kate runs the ACO REACH program for Optum, merging payer and provider capabilities to support further innovation in operationalizing innovative payment models. She is currently pursuing an MBA from The Wharton School, graduating in May of this year.
