Payer and Provider Strategies for Last Mile Care

In healthcare, the last mile is the link between the consumer and where care is delivered. Bridging the last mile in healthcare is paramount but often a barrier. When consumers can’t pass through the last mile connection to the healthcare delivery system, they never even get an opportunity to engage with healthcare up close.

Solving the last mile problem is not only about improving access but also about looking at consumers in a new way and creating a whole new business model to engage them in their care. Our panel will share success stories and best practices for closing the last mile with healthcare consumers.

 

Dianne Davis

Vice President, Community Wellness & HomeMeds, Partners in Care Foundation

Dianne leads a team of professionals and volunteers in implementing a diverse portfolio of evidence-based programs. She has 30 years of experience in healthcare administration, Managed Care and Gerontology. She has been with Partners for nearly ten years, overseeing Community Wellness’ contracts for evidence-based programs with federal, state, county, city, health systems and private foundations.

Dianne holds an MPH from UMASS Amherst and a post-graduate certificate in Gerontology from UMASS Boston. She speaks at numerous conferences, is Secretary of the Evidence-Based Leadership Collaborative, is a mentor for the NCOA Network Development Learning Collaborative and taught a course at UCLA, Evidence-based Programs for Older Adults.

Mitchell Fong

Vice President, Virtual Care, Renown Health

Mitchell is a certified public health professional who has developed telemedicine for underserved communities and holds dual master’s degrees in public health and business administration. He has been in the healthcare industry for over ten years. Mitchell leads virtual care at Renown Health, a multi-hospital system that is locally governed and locally owned. Renown is a not-for-profit integrated healthcare network serving northern Nevada, Lake Tahoe and northeast California and was an adopter of telehealth in 2012. Renown’s vision is to make a genuine difference in the health and well-being of the people and communities they serve.  As a public health professional, Mitchell has developed population health and virtual care strategic plans for the community. He is dedicated to his mission of improving the community’s healthcare, no matter the size or barriers.

Rhonda Wiering

Clinical Growth & Innovation, Avera at Home

A healthcare executive and clinical operations director with 25+ years of health care experience, Rhonda has expertise in hospital/ long-term care/ medical clinic/ home care/ hospice operations, regulation, budgeting, quality and information technology. Currently, she is responsible for strategic leadership for clinical growth and innovation strategy within the Avera@Home enterprise.

Rhonda is a Registered Nurse licensed to practice in Minnesota, Iowa, North Dakota, South Dakota and Nebraska. She received a Bachelor of Science Degree in Nursing from South Dakota State University, a degree in Healthcare Administration from the University of Minnesota, Carlson School of Business and a Master of Science in Nursing, Leadership and Management from Walden University.

Daniel Rivas

Senior Manager, Community Health, Blue Shield of California

In his role, Daniel Rivas drives Blue Shield of California’s population-centered work building community-relevant solutions to address childhood trauma, access to behavioral health services and the development of Social determinants of health (SDOH) programming that have a major impact on our members’ health, well-being, and quality of life.

Daniel holds a Bachelor of Science and a Master of Arts in Organizational Leadership. With over two decades of experience working for and with some of the country’s hard-to-reach populations, Daniel has effectively translated the vision, mission and core objectives of several institutions into significant results in engagement, reach, impact and delivery of quality services to some of our most disproportionally impacted communities.
